Insurance

It is your responsibility to know if our office and providers are participating with your insurance company and plan for all services, including laboratory. As a courtesy to our patients, we do file most insurance claims. You will be asked to pay for any office co-payment at the time of arrival for your schedule visit. You may be asked to pay for remaining account balances also.

You will also be responsible for any portion of the bill remaining after your insurance has paid their part. Please remember that regardless of whether the claim is filed by you or by our office, the bill is ultimately your responsibility. When we file a claim for you, we will make every effort to collect from your insurance company. However, unless we have payment or reason for a delay from them within 30 days of filing (as required by insurance companies under Georgia law), we will look to you for payment. To assist in the office efficiency, please present your insurance cards at your every visit. We are also required to obtain a copy of a picture ID for our records. If at any point your insurance information changes, please inform our front office staff immediately.

Payments

We encourage you to ask questions regarding our services or fees. We accept cash, checks, Discover, Visa, AMEX (American Express) and MasterCard as payment. Payments on your account must be mailed to the address given on your monthly statement. Payment plans are not encouraged, but when necessary for established patients, they must be arranged with our billing service prior to visits or procedures. Forms Completion

We require 5 business days to complete requests for forms completion. We will notify you when the form has been completed. There may be a pre-payment required to complete forms or write letters on behalf of the patient.
