We see patients by appointment only. However, we try to be flexible for urgent patient needs. Our office hours are 8:30 a.m. to 5:00 p.m., Monday through Friday. We schedule appointments between 8:30 a.m. and 4:00 p.m. We make every effort to stay on time, realizing that your time is valuable too. We do not address walk-in's at this time. Please note that if you are more than 20 minutes late for your appointment you may be asked to reschedule.
A "new patient" packet of information was sent to the address provide to our office by your referring physician. Please complete the enclosed paperwork prior to your visit and bring it with you to your scheduled appointment. We have enclosed a Registration Sheet, Consent and Authorization Form and a Comprehensive Patient Questionnaire for you to complete. Please carefully complete all questions to the best of your knowledge. Failure to do so may require us to reschedule your appointment. Please do not mail your paperwork back to our office!

Many of our patients are referred by their primary physician or specialist for evaluation and treatment. Our doctors work closely with referring physicians and provide them feedback on a regular basis. Some referred patients may be seen for long-term or follow-up care; however, the referring physician remains the patient's primary physician.
Secure parking is provided in the YELLOW parking deck adjacent to our building. Please bring your parking ticket with you to the office so the staff can validate it. If we can not validate the ticket you may be charged a parking fee.
