Surgical Critical Care and Acute Care Surgery Fellowship Program

The one-year ACGME-accredited surgical critical care fellowship training program at Atrium Health Navicent began in 2010 with one fellowship position. The program expanded to two fellowship positions in 2013.

SCC fellows are always closely supervised with one-on-one interaction with the attending in the ICU. This includes daily rounds and bedside procedures.

The fellows will rotate each month through different intensive care units. All units run at near full capacity with a very high acuity level. There is an average daily census of more than 50 mechanically ventilated patients. In addition to mechanical ventilation many of the patients require hemodynamic monitoring, vasoactive drug support and continuous renal replacement therapy or standard hemodialysis. The clinical experience for the fellow if board based with core rotations in Surgical Trauma ICU (6 months), Cardiovascular ICU-A (1 month), Cardiovascular ICU-B (1 month), Neuro ICU (2 months), Nephrology (2 wks), Infectious Disease (2 wks), Palliative Care (2 wks), and Advanced Echocardiography (2 wks).

The fellow is expected to complete the Critical Care of the Surgical Patient weekly curriculum and will meet regularly with PD to track progress. This curriculum is provided and consists of weekly lessons and quizzes starting August 1st through July 31st.

The fellow is expected to participate in all lectures and assigned Sim Lab sessions. This is protected time for the fellows as attendance is mandatory.

The fellows are to participate in a research project and give a minimum of one lecture at grand rounds or M&M.

The fellows receive an educational stipend of $1,500 per year for the purchase of books, journals, or the attendance to educational conferences such as the Society of Critical Care Medicine, etc.

There are goals and objectives based on the six core competencies for each unit. These are further broken down into knowledge, skills, and abilities in the principal components of Cardiovascular Physiology, Pathophysiology and Therapy, Renal Failure, Respiratory Disorders, Pharmacology, Gastrointestinal Disorders, Endocrine Disorders, Ethics and Palliative Care, Hematologic Disorders, Infectious Diseases, Monitoring and Bioengineering, Neurology, Nutrition, Pediatrics, and Trauma. The fellow is responsible for maintaining a portfolio and this will be reviewed with the program director.

A Message from the Surgical Critical Care Program Director

For the academic year of 2025-2026, the Surgical Critical Care (SCC) Fellowship transitioned to a 2-year training program. Fellows complete 1-year of Surgical Critical Care training followed by a second year in Acute Care Surgery (ACS). Our Acute Care Surgery year received the esteemed American Association for the Surgery of Trauma (AAST) endorsement. The goal of this 2-year fellowship program is to train well round surgeons in the disciplines of critical care, trauma, and emergency general surgery. The 2025-2026 fellowship class will be the first to complete this training platform. Drs. Bill Wallace and Kristin Harmon are the Assistant Program Directors of the SCC year and ACS year, respectively. Additionally, our department welcomed a new SCC/ACS faculty member in Dr. Roslyn Alexander as well as a new Fellowship Coordinator in Brittany Smith. Dr. Alexander’s passion for teaching and Brittany’s interest in program development will be much appreciated.
