Navicent Health Baldwin Partners with AmeriMed for Post-Acute Care
Access to Care After Hospital Discharge Crucial for Patient's Continued Health
Atrium Health Navicent Baldwin (NHB) has partnered with AmeriMed Mobile Integrated Healthcare to provide access to in-home care and assistance following discharge from the hospital. Studies have shown that patients who do not continue with follow up care within seven to 14 days of discharge from an acute care hospital face increased chances of becoming ill again, requiring urgent or emergent care, and hospital readmission. By assisting patients after discharge, NHB and AmeriMed hope to improve each patient's odds of a successful outcome.
"When patients are ready to be discharged from the hospital, that does not mean the end of their care, or the end of their journey toward wellness. In fact, it is critical that their care continue once they're discharged from an acute care hospital. Through our partnership with AmeriMed, we not only improve health outcomes but also decrease mortality," said Douglas Brewer, MD, Chief Medical Officer of NHB.
Qualifying patients who are discharged from NHB will be referred to AmeriMed for post-acute care. An AmeriMed care coordinator will contact the patient to schedule a follow up visit at the patient's home. The patient's AmeriMed care team will review the patient's discharge plan, assist with scheduling any necessary follow up appointments, review prescriptions and ensure medication adherence, and also provide a risk assessment of the home to address any hazards.

We want to learn what their needs are, we want to take the time to answer any questions they may have, and we want to make sure they have a clear understanding of their next steps on the journey to health and wellness.
