Atrium Health Navicent At Home is an integral part of the healthcare delivery system of Atrium Health Navicent and serves to meet the patient's needs in their home. Through comprehensive home health care services, our expert staff works one-on-one with patients and families to promote patient independence through a continuum of care.
The duration of a patient's stay on home health services can vary depending on several factors, including the patient's condition, the type of care needed, and the coverage under their insurance plan. However, here are some general guidelines:
- Medicare: Under Medicare, home health care is provided if the patient meets certain eligibility criteria. Typically, a patient must be under the care of a physician, be homebound, and require skilled care (e.g., nursing, physical therapy). The duration of care is based on the patient's progress and needs, and Medicare will generally cover home health services for as long as these conditions are met. Coverage is usually reviewed every 60 days, with potential for renewal if the patient's condition warrants continued care.
- Private Insurance: Coverage can vary widely based on the individual insurance plan. Some insurance policies may cover home health care for a set period, while others may provide coverage based on medical necessity. Patients often need to meet specific criteria for extended coverage.
- State Programs: For those covered by state Medicaid programs, the duration of home health services can depend on the state's guidelines and the patient’s specific needs. Coverage is often based on the severity of the medical condition and other factors such as the patient's ability to perform activities of daily living.
- Medical Necessity: Regardless of the payer source, home health services are typically provided if they are deemed medically necessary. If a patient’s condition improves and they no longer require skilled care, home health services will be discontinued. On the other hand, if a patient's condition worsens or requires ongoing skilled services, care can be extended.
In addition to above, to qualify for home health services, the patient must be 18 years of age or older, have a willing and able caregiver to participate in their care, and have had a face-to-face visit with their physician within 90 days prior to the home health referral.
