Privacy Notice/Code of Conduct


HIPAA is the acronym for the Health Insurance Portability and Accountability Act of 1996. The HIPAA Privacy Rule requires hospitals and other covered health care providers to develop and distribute a notice that provides a clear, user friendly explanation of individuals rights with respect to their personal health information and the privacy practices of the health care provider. Our Non-Discrimination Policy

Advocate Health follows all State and Federal Civil Rights laws. We do not exclude or treat anyone unfairly (discriminate) based on:

  • Gender Identity

This notice applies to anyone receiving care in the programs, services and activities at Advocate Health. This notice applies to all people in our facilities. Advocate Health:

  • Offers free aids and services to people with disabilities to communicate with us, such as, but not limited to:
    • Qualified sign language interpreters
    • Written information in other formats (large print, audio, accessible electronic formats, other formats)
  • Provides free language services to people who do not speak English as their first language. These services include:
    • Qualified medical interpreters
    • Information translated into other languages
  • Does not ask people to use minor children, family members, or friends as interpreters.

If you need these services at an Advocate Health location, tell a staff member.

How to File a Service Complaint

You can file a complaint in person, by mail, or email if you believe that Advocate Health did not provide these services or if we treated you unfairly (discriminated). If you need help filing a complaint, please speak with a staff member:
